Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

PLEASE READ CAREFULLY

Congregation Darchei Noam has put in place preventative measures to reduce the spread of COVID-19 at its in-person services and similar programs. However, attending the services or similar activity could increase your risk and your child(ren)’s risk of contracting COVID-19.

I understand that this Assumption of the Risk and Waiver of Liability agreement is in addition to, and is not a substitute for, any other documentation or registration information that I might be required to execute by DN in order to attend DN in-person Services in the DN’s Building or similar activity.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily agree to assume the risk that I and my child(ren) may be exposed to or infected by COVID-19 by attending these services and programs and that such exposure or infection may result in illness from COVID-19.

I HEREBY RELEASE, COVENANT NOT TO SUE, DISCHARGE, AND HOLD HARMLESS THE CONGREGATION DARCHEI NOAM, ITS BOARD, OFFICERS, EMPLOYEES, AGENTS, AND REPRESENTATIVES, OF AND FROM THE CLAIMS, INCLUDING ALL LIABILITIES, CLAIMS, ACTIONS, DAMAGES, COSTS OR EXPENSES OF ANY KIND ARISING OUT OF OR RELATING THERETO.
